Once we have received these 3 forms we will send you a catalog and swatch book and email you with your new account number.

The opening order minimum amount is $350. The minimum re-order amount is $125. Re-orders placed under this minimum will incur a $10 fee. We accept Visa, Mastercard, and American Express.
